What to Expect
Before your first visit, you will receive digital forms through email or text message including a medical and dental health questionnaire and privacy form. Please complete these forms using your phone or computer. If you are unable to complete these forms prior to your appointment, please arrive 10 minutes early to your appointment and we will be happy to assist you completing these forms in our office.
Your first visit is for consultation only. Your periodontist will examine the health of your gums. We will make every attempt to obtain updated x-rays from your other dentist or dental specialists before your appointment. If additional x-rays are required for diagnosis, they will be taken at your appointment. The Periodontist will review with you a diagnosis and treatment recommendations.
Our administrative team will provide you with a treatment plan and estimate. If you have dental insurance, we will submit your treatment plan for predetermination of benefits to your carrier.
Your referring dentist will be provided a full report as well as copies of any x-rays. We will also provide them updates as you progress through your treatment.
Please plan to be in our office for approximately 45 minutes for your initial visit. If you have pain or an urgent situation, we will make every attempt to see you that day.
We value you and your time and will do our best to see you promptly at your scheduled time. If you need to change your appointment, please provide us at least 2 business days notice.
Payment is due in full at the time of treatment. For your convenience, we accept payment by cash, debit, MasterCard, and Visa. We do not accept direct assignment from your dental insurance carrier; however, we will complete and submit a claim on your behalf for reimbursement to you. When possible, we will submit your claim electronically.
